Everyone needs to fill out the Activity Medical Release Form. Those of you that are under 18 years of age will fill out the top form and those of you that are over 18 will fill out the bottom form. Thank you and we look forward to a great year.

We will be attending Summer Camp @ Riverbend Retreat Center. In order to get your name on the list ; you must turn in forms and money to Josh or Duff.
